Background Investigation Information

Congratulations!

You just passed your written and physical fitness test! The next stage of the hiring process will be backgrounds. The background process will start in just about two weeks. Your investigator will send you an email inviting you to complete the online personal history questionnaire (PHQ) through our internal system. This email will inform you that you must complete the PHQ, polygraph booklet, and applicable waivers within TEN calendar days, or you risk being involuntarily withdraw from the selection process.

The following information is what you will need to provide to your investigator. Take this time to start gathering documents to save you time. Remember! You only have TEN days to complete this packet:

• State issued driver's license
• Social Security card
• Birth certificate
• Proof of US citizenship
• High school (or GED) and college transcripts (must be officially sealed)
• Military DD-214 (if served) or Statement of Service (if active)
• Selective Service Card (males up to 25 years old)
• All legal name change documents
• All civil and criminal court documents
• 10-year social security work history (do not purchase this document until instructed by your investigator)
• IAB clearance letter (if current law enforcement)

Your investigator will also require the following character information:

• Places of residency from the age 15 years old
• The last 10 years of employment (name, address, and supervisor contact information)
• Contact information for references (name, address, phone number, email)
• Prior law enforcement agency applications (if applicable)
• Motor vehicle information: all states where you have been licensed to operate a motor vehicle, all traffic citations, and all motor vehicle accident (whether at fault or not)
• Financial information
